Plan Options
- Adult Plan: Designed for adults who will benefit from regular preventive care.
- Child Plan: Designed for children up to age 14.
- Perio Plan: Designed for patients with periodontal concerns who will benefit from three periodontal maintenance visits per year.

This is an in-office program, not an insurance policy. Any unused services do not roll over into the next plan year. Services are non-transferrable. The plan cannot be purchased with CareCredit® and plan discounts are not applicable when CareCredit is used to pay for treatments.
